How is Management of Diabetes supported at BGPN ?

Management of Diabetes includes facilitating the expansion of diabetes services, increasing the role of the practice nurse to better manage diabetes and promote awareness of what the program will provide. The practices currently running diabetes clinics are seeing improvement in diabetes management and better use of resources available to patients with

BGPN provides support through the following services :

• Assistance with planning & implantation of Nurse led diabetes clinics
• Assist practice to maximise MBS item number utilisation through optimal data management
• Assistance with population identification and ratification [recalls & reminders]
• Improving linkages with local allied health providers
• Access to Diabetes education services
• Peer support and CPD opportunities for local DE network
• Providing opportunities for upskilling & updating clinical knowledge for GPS , Nurses and AHPs
• Actively supporting a regional base to interconnect diabetes services in the South east / Bayside area [SEBDA]
